Claim Notice Forms
 
For providing notice of claims: Please call your insurance company's primary claim reporting number, and Ellett Insurance as backup. If either is unavailable:
1) Please print and complete applicable claim notice form (Numbers denote each Acord form number).

2 - Automobile Loss Notice
3 - General Liability Notice of Occurrence /Claim
4 - Workers Compensation 1st Report of Injury
 
2) Consult Claim Reporting Phone Numbers and Addresses if unable to reach Ellett Insurance. Identify your insurance company and fax a completed claim form directly to them with a copy to Ellett Insurance. Be sure to let Ellett Insurance know you have sent the form directly to your insurance company so the agency does not duplicate your efforts, but can follow up on your claim.
